If you are an existing or previous client, you may need me to send an evaluation to your current doctor, school, or mental health professional. Please complete the following release of information form, this gives me permission to release this confidential information. You may then fax it to: 972-572-9320.
You do not need to print the HIPPA policies out (unless you want to). There is a page in the new patient paperwork that you will sign regarding HIPAA.
