In order to begin your appointment on time, you could complete paperwork ahead of time!  Please download, print, and complete the following form.  If you have questions about any portion, we will discuss on your first visit.

If you are an existing or previous client, you may need me to send an evaluation to your current doctor, school, or mental health professional.  Please complete the following release of information form.  This gives me permission to release this confidential information.  You may then fax it to:  972-572-9320.

Dr. Blandino's HIPAA Paperwork: You do not need to print this out (unless you want to).  There is a page in the new patient paperwork that you will sign regarding HIPAA.
